Student Physical Examinations

Athletes in school-sponsored sports must have a current sport-qualifying physical exam on file in the Activities Office. Physicals are good for 13 months from the exact date of the last physical.

Once you have your physical completed by a doctor, you can scan it/take a photo (of each page) and upload the completed forms on the Activities & Athletics Registration Website. If you cannot access technology to upload the physical forms, you can turn them in at the building office.
